From the research, they listed 69 categories of human errors that could occur during an operation. Some of those errors include experiencing overconfidence during a procedure, hyper-focusing on less important details and having lapses in decision making.

When operational malfunctions do occur, these are some of the most common:

  • The surgeon leaves a piece of equipment inside the patient.
  • The surgeon makes an incision in the wrong part of the body.
  • The patient receives the wrong organs during a transplant.
  • The patient gets an operation they don’t need.
  • The patient falls off the operating table because they were not appropriately secured.
  • The patient wakes up in the middle of surgery due to lack of anesthesia.
